Here’s what you need to know for your in-person sessions
Building - Look for the grey 2 story house with white garage door.
Parking - Park in the driveway or on the street (please do not block the mailboxes).
Entry - To know if I’m ready for you, please check to the left of the garage door for a green sign that says “welcome, please come in". If the sign is out, come on in! If it’s not, I’m not yet ready for you OR this may be a good time to double check your appointment day and time. 😉
Inside - Once you step inside, straight ahead of you will be an open or closed white door. If the door is open, feel free to come into the studio. If the door is closed, you may wait in the entry way or wherever you feel most comfortable.
Note: There are coat hooks on the wall in the entryway. If you'd like, you may hang anything you don’t need during your appointment here.

I believe in providing each and every individual the highest quality of service throughout your course of treatment.
I believe the highest care is delivered in a one-on-one setting, so you’ll see me for each treatment and will never be passed off to an aide or tech throughout the course of your care.
I take the whole individual (body and mind) into consideration with each and every treatment being highly individualized to meet your specific needs and guide you as you work toward your unique goals.
All treatments/sessions take place in a private treatment and gym space.

No, you don’t!* Oregon is a “Direct Access” state meaning you do not need to see a physician prior to being seen in physical therapy.
If I believe you need to be seen by a physician, I will refer you to one.
*Note: Some insurance companies/plans require a physician referral or prescription for coverage of physical therapy services. It is always in your best interest to contact your insurance to determine your specific physical therapy coverage.
-
Please wear comfortable clothing that is easy to move in and will allow for easy access to the area of treatment.

Physical therapy is a cost-effective treatment option for overcoming pain and improving function. Many insurance plans cover the majority of your physical therapy treatment, though you may be responsible to pay a percentage (copay or co-insurance) or deductible.
I’m currently contracted as an in-network provider with the following insurance companies:
BlueCross BlueShield of Oregon
PacificSource
It is always in your best interest to contact your insurance company to understand your specific physical therapy benefits.
-
You may contact me and I will happily do my best to help you determine your insurance coverage.
You may also reach out to your insurance company through member/customer service to better understand your individual benefits.
-
Most likely, yes.
If I’m not contracted with your insurance company, I offer Self-Pay pricing.
Upon request, I will provide you with a detailed receipt called a “superbill” which you can submit to your insurance for “out-of-network” reimbursement. The best way to understand your out-of-network benefits is to call your insurance company and use this SCRIPT to assist you with collecting benefits information.
See above details regarding self-pay rates.

I am not a participating provider with Medicare, therefore I cannot provide you with physical therapy treatment.
That being said, I would be more than happy to direct you to a physical therapy provider accepting Medicare in your area.
If you are seeking coaching or would like to work with me to improve your mobility or strength I would be happy to see you on a self-pay basis.
Please contact me to discuss what you are looking for and how I might be able to help.
